Normal check-up. Patient is healthy and has no signs of mental or physical health problems. Nothing important to note.
December 1994 - Royal London Hospital
Patient is diagnosed with chicken pox. Prescribed herbal bath treatments, anti-itch cream, and Acyclovir. Follow-up call one week later. Parents said he had made a full recovery with no signs of complications.
August 1995 - Royal London Hospital
Patient complains of vivid nightmares and fitful sleep. After a session with the the hospital's psychologist, patient was diagnosed with sleep behavioral disorder. Prescribed a mild sleep aid, and psychologist suggested the patient take up a hobby. Patient was otherwise in perfect health, and encouraged to participate in more sessions with the psychologist.
April 1996 - Royal London Hospital
Patient visited with psychologist again. Patient took up acting over a year ago and has seemed to help the patient tremendously with his sleeping problems. Patient encouraged to stick with his hobby and was taken off of his sleep medication. Parents were asked to watch the patient for any signs of relapse.
June 2001 - Chicago University Medical Center
Normal check-up. Medical records transferred from Royal London Hospital to Chicago University Medical Center. Patient is in good health. Nothing important to note.
July 2001 - Chicago University Medical Center
Patient is again suffering from horrific nightmares and fitful sleep, accompanied by violent daydreams and hallucinations. Attempted to prescribe medication, but was refused by the patient's parents. Although deprived of good rest, patient was otherwise in good health.
August 2010 - Chicago Metropolitan Correctional Center Infirmary
Patient was found on scene at his home, which was on fire as a result of an explosion. Patient was unconscious and covered in cuts, scrapes, bruises, and burns. Placed in Correctional Center Infirmary due to being a prime suspect to the fire. Patient was treated and spent a month in recovery and rehabilitation.
September 2010 - Chicago Metropolitan Correctional Center Infirmary
At the court's request, patient was given a full psychological evaluation. Patient still suffering from violent dreams and hallucinations. Subjected to hypnotherapy, and was discovered to have a split personality that has named itself "Zander." Patient seems to be unaware of the personality and cannot recall any actions that the personality performs. Diagnosed with Dissociative Identity Disorder, and scheduled for regular therapy after case is resolved.
October 2010 - Anaktuvuk Pass Health Clinic
Medical records transferred from Chicago University Medical Center to Anaktuvuk Pass Health Clinic. After another psychological evaluation, patient was prescribed with Zuclopenthixol for DID, Paxil for depression, and Lorazepam for sleeping problems. Also set up for mandatory therapy with the psychologist.
